Job Descriptions Overview DHAP Case Manager - Bi-lingual (Spanish) Description: The DHAP Case Manager is responsible for managing participants' disaster needs by conducting interviews in person and via telephone. Effective communication skills to communicate program guidelines, needs assessments and follow through is essential. The ability to accept responsibility for maintaining participants' file, interaction via phone and home visits to ensure individual development plans (IDPs) are met. The Case Manager must possess the ability to access participants' needs clearly to provide assistance and be able to properly refer social service agencies for unmet needs.Skills/requirements: Ability to communicate, read and write in Spanish is essential. Excellent customer service skills are a must in addition to the ability to deal with difficult situations in a diplomatic and professional manner. The ideal candidate will have excellent computer skills; with advanced Excel spreadsheet experience required. Working knowledge of word is essential. Education/experience: Bachelors degree in Social Work preferred or Associates degree with a minimum of 2 years experience in Social Services or Case Management. This is a grant-funded position, which terminates upon closure of the program or loss of funding directly supporting the DHAP program.
